You will not be required to mention either the FOI Act or the Data Protection Act or give a reason for your request when making a request for information.

Before the Trust is required to respond, a request will have to satisfy certain conditions:

• In a permanent form, for instance in writing or by e-mail 
• State the name of the applicant, address for correspondence 
• Include sufficient information to enable the Trust to identify the information requested
