NHS Funded Referrals

Bristol Centre for Reproductive Medicine welcomes NHS funded patients for assessment and treatment from all referring locations. The fertility policy of your local Clinical Commissioning Group (CCG), will determine your eligibility for access to NHS funded fertility assessment and treatment at BCRM. Please refer to your CCG locality website or contact your GP for further information.

To ensure acceptance of your referral your GP will need to include:

  • The eligibility paperwork specific to your CCG (Confirmation letter of funding approval or The Fertility Assessment Application Form (Ticklist) for BNSSG Patients)
  • A referral letter detailing your patient history and that of your partner (if your partner is registered at an alternative GP please ensure their information is forwarded to us)
  • Preliminary blood test results

Please note; if preliminary blood tests, a referral letter or NHS funding confirmation are not included this will result in a rejection of your referral.

Bristol, North Somerset, South Gloucestershire (BNSSG) referrals

NHS funding for BNSSG patients is subject to the access criteria outlined in the Fertility Assessment and Treatment Policy statement (December 2014 Version 1516.1).

Completion of the Fertility Assessment Referral Form by your GP is required as part of your referral to BCRM to ensure you meet the criteria for NHS funded care. BCRM are unable to accept NHS funded referrals for patients within BNSSG that do not meet all of the criteria outlined in this form.
