Exceptional Healthcare Awards - Patient Safety Champion

There is just one week to go until North Bristol NHS Trust’s annual Exceptional Healthcare Awards.
The awards, supported by Southmead Hospital Charity, celebrate the efforts and achievements of some of our outstanding members of staff and volunteers.
In the lead-up to the awards ceremony on November 11 we will be showcasing the shortlisted nominees in every category.
Today we are starting with the Patient Safety Champion category, which recognises teams or individuals who have improved patient safety and / or reduced risk either in a defined area or across the whole organisation.
The shortlisted nominees are:
Patient Safety Medicines Management Team
Medication errors cause harm to patients and can increase healthcare costs. This team’s efforts to improve patient safety in medicine management have been recognised in a Pharmaceutical Society Fellowship.
Principal Pharmacist, Jane Smith, said: “Patient safety is the main focus of the work we do within the Medicines Management Team and is something we are incredibly passionate about.
“Our work involves Nurses and Doctors as well as all Pharmacy staff and we are delighted that our efforts have been highlighted by our colleagues through the Exceptional Healthcare Awards.”
Emergency Department (ED) Team
The ED team is always looking for ways to improve patient safety and experience including the ‘Shine’ care scheme which has been designed to improve patient safety and care documentation.
The implementation of this new process was led by Emergency Department Consultant Ben Jordan alongside Senior Staff Nurse Karl Smith and Abi Middel with support and participation initially from all senior nurses and doctors within the department, followed by the rest of the team.
As a result of this the team have been commended by the CQC for quality improvements.
The nomination said: “All of this has been possible due to the resilience and passion from the ED team. The team really have demonstrated that they are patient safety champions.”
Nikki Davies, Lead Diabetes Specialist Nurse
Always the patient’s advocate, Nikki has promoted patient safety through incident reporting, developing clear care pathways and by cascading and sharing best safety practice.
Nikki said: “I’m really happy to receive this nomination; I really didn’t expect it. It’s so nice that people see that you work hard and do extra. It’s great to be recognised for this.”
